Turning on Compatibility View in Internet Explorer 8


If you are using Internet Explorer 8, and the forms don't appear after you click select a forms tab, then please follow the following steps:

1. Navigate back to the Forms page on our website.
2. Move your mouth curser to top, right-hand corner of your Internet Explorer 8 (IE8) window.
3. Click on the drop-down menu titled "Page".
4. Move halfway down the menu and click on "Compatibility View".
5. Your IE8 will now reload the page.
6. Click on one of the form tabs (such as "New Patient Forms").
7. You now should be able to see links to each PDF form.
